How our service can help you
We provide care and support for people, aged 18-65, who have severe mental illness. We care and support people who live in the London Borough of Croydon. People eligible for our service must be on the Care Programme Approach (CPA). The CPA is a way of describing what you need in order to get the best care. It is for patients who need to see several people or organisations for their mental health care and treatment. We provide people with a care co-ordinator to plan their care and make sure they get the best available help. They are usually a community psychiatric nurse, social worker or occupational therapist. We also run an outpatient clinic for people who are able to live more independently. Our service brings together teams of health and social care professionals, including nurses, social workers and psychiatrists.
