Community Mental Health Service

Experts agree that people with dementia and long term mental illness are, where possible, best looked after in their own homes. Our Community Mental Health Teams (CMHTs)  provide support to anyone with dementia or people over the age of 65 who are experiencing mental health problems but don’t need to be in hospital. We also provide specialist services such as challenging behaviour, early intervention, memory and crisis teams.

Many things can cause mental health difficulties - physical or mental illness, past experiences, difficult relationships, drug and alcohol problems or any stressful situation that has an impact on your ability to cope in everyday life.

Community mental health teams

We have 4 community based mental health teams providing specialist assessment, diagnosis and treatment, across south east London. Each team is made up of experienced psychiatrists, community psychiatric nurses (CPNs), occupational therapists, social workers, psychologists, mental health support workers and administration staff. We work closely with other social care staff, voluntary and regulatory organisations to make sure everyone receives a level of treatment and care that  is appropriate, effectively co-ordinated to help maintain health and to live as independently as possible.

The level of contact and intervention may vary greatly depending on an individual's level of need and treatment plan.

We deliver care in:

  • People's own homes, including residential and nursing homes
  • Outpatient clinics
  • Community hospitals
  • GP surgeries

Community mental health teams work with carers and relatives, hospital liaison teams and GPs to decide how care and support can be given when patients are discharged from hospital.

We know that practical activities can really help, for example the Croydon CMHT is working on a reminiscence project for clients with dementia. They also run a summertime gardening club with the charity ‘Mind’.

Our Lewisham team recruit and train ‘recovery enablers’. These are volunteers who are able to commit to a minimum of four hours a week.  They work along side qualified staff to assist people on their recovery journey, whether that be ‘continuing to be me,’ managing their mental health difficulties or dementia.  We hope this will extend to other boroughs in due course.

There are also opportunities to take part in research studies which can help us explore new treatments.

 Home treatment team

We have 2 teams that offer short-term treatment for people over the age of 65 with severe mental illness. We understand that most people prefer to be treated at home with the support of family and carers, especially when facing a crisis. 

When you have been assessed by mental health professionals and need more intensive support, there are times when home treatment can be an alternative to going into hospital or make an early discharge easier.

Our services, one which covers the boroughs of Lambeth, Lewisham and Southwark (based at the Maudsley Hospital), and the other covers Croydon (based at the the Bethelm Hospital) areflexible and responsive as we work every day of the year. The teams consists of an administrator, nurses, support workers, social worker, occupational therapists and doctors. We work with GPs and other community mental health teams.

Members of the teams can help with practical everyday needs like shopping, getting meals ready or helping you to manage your medication effectively.  We can arrange occupational therapy services or psychological therapies and treatments. It may be an introduction to community services and resources would be helpful for example clubs, day centre and voluntary groups. If we don’t offer a service ourselves, we try to find a professional or organisation that can help and put you in touch with them.

Mental Health Care Home Intervention Teams

These specialist teams support patients who have particularly challenging behaviour who are aged 65 years and over with mental health problems and anyone with a diagnosis of dementia living in a residential or nursing home or who attend a day centre in Lewisham, Southwark and Lambeth.

They also provide training and support for care staff to develop their skills and knowledge to improve the quality of care in residential homes.
