Publication scheme

We routinely publish information about the organisation under the classes of information outlined in this Publication Scheme. This publication scheme has been adopted by the Trust without modification on 1 January 2009 based on the model scheme approved by the Information Commissioner's Office and will be valid until further notice.

This publication scheme commits the Trust to make information available to the public as part of its normal business activities. The information covered is classed in to seven categories, where this information is held by the Trust.

Rights of access to information which is not included in the Publication Scheme

In addition to accessing the information identified in this Publication Scheme, you are entitled to request information about South London and Maudsley NHS Foundation Trust under the Freedom of Information Act (2000). Our Freedom of Information Policy illustrates our procedure for dealing with information requests under the Freedom of Information Act (2000).

We will respond to your information request within 20 working days. Sometimes, some or all of the information requested cannot be provided. We will explain the reasons why not when this happens.

The Freedom of Information Act does not change the right of patients to the protection of their confidentiality in accordance with Article 8 of the Human Rights Convention, the Data Protection Act (1998) and common law.

Patient information

Maintaining the legal right to patient confidentiality is commitment with an upmost importance on our part. The Trust has appointed a Caldicott Guardian, who is a senior clinician with responsibility to ensure the protection of patient confidentiality throughout the Trust in accordance with your legal rights. Under the Data Protection Act (1998), you are entitled to access your clinical records or any other personal information held about you. You can make an application to the Data Protection Office by completing the Subject Access Request Form.
