Assessment and identification for young people at-risk for mental illness

You may be eligible for this study is you meet the following criteria: 
•    Ages 12-30 
•    Noticing a recent change in thinking, behavior, or experiences, such as: 

  1. Confusion about what is real or imaginary 
  2. Feeling not in control of your own thoughts of ideas 
  3. Feeling suspicious or paranoid 
  4. Having experiences that may not be real, such as hearing sounds or seeing things that may not be there 
  5. Having trouble communicating clearly 

Most people who have these symptoms do not go on to develop psychosis. We are trying to understand why some people have these experiences do develop psychosis while many others do not.

Inclusion Criteria

CHR subjects:
(1) Individuals between 12 and 30 years old, 
(2) Understand and sign an informed consent (or assent for minors) document, 
(3) Meet diagnostic criteria for CHR from the Positive Symptoms and Diagnostic Criteria for the CAARMS Harmonised with the SIPS (PSYCHS)
 
Healthy controls:
(1) Individuals between 12 and 30 years old, 
(2) Understand and sign an informed consent (or assent for minors) document

Exclusion Criteria

CHR subjects:
(1) Antipsychotic medication exposure equivalent to a total lifetime haloperidol dose of > 50 mg (see Table 1.0 in Appendix - Protocol), estimated based on available information (e.g., medical file documentation, participants and family report) or current antipsychotic medication at time of screening assessment
(2) Documented history of intellectual disability
(3) Past or current clinically relevant central nervous system disorder. When necessary, Research Assistants will consult with study team investigators (including medical personnel) to determine if the central nervous system disorder is deemed to be clinically relevant
(4) Traumatic brain injury that is rated as 7 or above on the Traumatic Brain Injury screening instrument
(5) Current or past treated or untreated psychotic episode, as determined using the PSYCHS.

Healthy controls:
(1) Documented intellectual disability
(2) Past or current clinically relevant central nervous system disorder. When necessary, Research Assistants will consult with study team investigators (including medical personnel) to determine if the central nervous system disorder is deemed to be clinically significant
(3) Traumatic brain injury that is rated as 7 or above on the Traumatic Brain Injury screening instrument
(4) Meet criteria for any CHR psychosis-risk-syndrome, any current or past psychotic disorder or Cluster A personality disorder diagnosis
(5) Must not be receiving any current treatment with psychotropic medication
(6) Must not have a family history (in first-degree relatives) of psychotic spectrum disorders.
