Pathway Homeless Team (Kings Health Partners)

How our service can help you

The team can support patients regardless of their local settled connection to an area in the UK, including those from abroad or without a connection in the UK

Patients are accepted if they meet all the criteria:
¥ 18+ years old
¥ Patients who are rough sleepers (not suitable for the START Team) living in homeless hostels, B&B, sofa surfers or who have nowhere to go on discharge
¥ Patients with any form of mental illness
¥ Regardless of a settled local connection
¥ Patients currently admitted to a Lambeth or Southwark hospital ward

The team doesn't usually work directly with patients who have a care coordinator either in the community or on admission. They will be happy to give advice and signposting where this required as we appreciate that situations and services can be complex and challenging to navigate.
