Community mental health survey 2025 – give us your views and help us to improve services
The NHS Community Mental Health Survey is now underway.
If you used our community services in April or May 2025, you could be invited to take part and may have already received the survey in the post, or a reminder by text message. You can complete the survey either as a paper questionnaire or online using the login details or QR code provided in the letter.
The survey is part of a national programme coordinated by the Care Quality Commission (CQC) to improve the quality of care and patient experience, by collecting the views of thousands of people using community mental health services across the country.
Taking part in this survey is voluntary and all answers are confidential. Service users have been selected at random as this is an invitation only survey and if you are selected to take part, your contact details will be used by researchers to carry out the survey. By completing the survey, you’re helping us understand what we’re doing well, and where we need to improve.
Feedback from patients is vital and tells us their views and priorities for bringing about improvements in the quality of care. The results help us see where we perform well and to identify any areas where there is room for improvement.
The survey is confidential, carried out by the Care Quality Commission and a dedicated Survey Coordination Centre on behalf of NHS England. Your individual responses are not shared with our staff, so participation will not affect your care or treatment.
The survey closes on 28 November and the results, which will be published in the Spring of 2026, will be used by the Trust to identify areas for improvement, and by CQC to assess Community Mental Health Trusts in England. If you have any questions about the survey please email: PEDIC@slam.nhs.uk
