How our service can help you

The Mental Health Care Home Intervention Team (Lambeth and Southwark) supports people aged 65 and over, whose behaviour has become challenging in the context of a dementia or mental illness and currently living in a day care, residential or nursing home setting and extra care sheltered.

Our aim is to provide a community in reach programme for older adults.

We work on the understanding that community providers have a responsibility to deliver the level of care necessary to meet the needs of people who challenge services. We provide a structured and collaborative assessment and formulation of the behaviour as well as providing learning opportunities for care staff.

The service supports care home staff in care planning including the deployment of non-pharmacological management strategies and opportunities for engagement in meaningful activities. We also provide process consultation and advice on organisational development for care staff and managers if appropriate.

We aim to complement other community health services and work collaboratively with other agencies for the benefit of service users and their carer and to avoid any unnecessary hospital admissions.
