Primary Care Mental Health Support Service (Croydon)

How our service can help you

The Primary Care Mental Health Support Service (PCMHSS) works with Croydon adults residents (18-65 years) that have experienced severe and enduring mental illness and have achieved some stability in their recovery that they are ready for transfer to Primary Care. The client group will be motivated to increase self management and independence in preparing for transfer to primary care. The PCMHSS works in collaboration with the patients, carers/family and their GP practices towards full transfer from secondary mental health service to Primary Care. Our team brings together mental health practitioners including a psychiatrist that will support the transfer process.

Patients accepted into the PCMHSS will meet certain criteria e.g. their mental health (mainly psychosis) and social situation is stable, and the prescribed medication can be managed by the doctor's surgery.

Patients will be transferred from the service, to primary care, following a formal multi-disciplinary review with the prescribing GP, practice staff and our service.

In the event of needing input from secondary service, the referral process will be the same as it is now for all.
